You can apply for a birth certificate if you’re an individual requesting your own birth certificate, the parents listed on the birth certificate, a court-appointed guardian of the person on the birth certificate, a surviving spouse of the person listed on the birth certificate, or a representative of the person listed on the birth certificate, but they have to have a signed letter by the individual, his parents, or his guardians.

You will need to have a legitimate government-issued photo ID. If you can’t provide photo identification, then you need to have some alternative forms of identification. You need two pieces of alternative documentation. They have to be two different types of identification too. At least one of the documents has to have the individual’s mailing address. The certificates will be mailed back to the address on the identification document. Some of the alternative forms of documentation you can use include a copy of income tax return/W-2 form, utility bill with current address, lease or rental agreement, a letter from a government agency that’s asking for a vital record, a statement from a bank, a current car registration, or a pay stub. You have to show that you have a real place of residence that’s verified who you are. Maryland gives you a lot of options in what you can use for identification, and there might be some other states don’t have those same options. You’re free to scrounge up some alternative identification if you don’t have a government-issued photo ID for some reason.
